Chronic Kidney Disease (CKD)is a progressive condition in which the kidneys are unable to function effectively. Over an additional 20 million Americans – 1 in 9 US adults – have CKD, and are at increased risk. CKD is commonly due to complications of diabetes and high blood pressure; people with both are especially at a higher risk.

- CKD Navigator patients receive a thorough orientation on each medication
- A Patient Care Coordinator calls the patient to determine adherence and assist with any therapy issues
- Prescription utilization reports, available to healthcare professionals, provide real-time pharmacy data and validate adherence
- Patient confidentiality is maintained at all times throughout the CKD Navigator Program
Financial Assistance
Financial assistance is available for patients who meet annual benefit maximums, are among the working poor or on disability and financially unable to burden their co-payments or co-insurance liabilities. No additional costs are ever passed on to patients aside from routine co-pays, deductibles, co-insurances, etc. Prescriptions are discreetly shipped or delivered throughout Michigan and the U.S. at no additional charge.
