GENENTECH PULMOZYME ENDOWMENT
Call Diplomat Specialty Pharmacy toll free at: 1.866.754.3639 to fill your Pulmozyme prescription. We will coordinate delivery to suit your needs.The Endowment’s purpose is to expand CF patients access to Pulmozyme therapy. It is funded by Genentech, but is operated by an independent Board of Directors with management provided by an outside firm.
You may be eligible:
- If you have insurance, but it is a policy with limitations such as Pre-existing conditions, etc
- If you have a co-payment too high based on family income
- If you are not eligible for alternative insurance sources
- If you have more than one patient on pulmozyme in a single family
AXCAN COMPREHENSIVE
How the Program Works: A person with Cystic Fibrosis is placed on ULTRASE® (pancrelipase) or ULTRASE® MT (pancrelipase) therapy.The person with CF mails a proof-of-purchase of one month’s supply of ULTRASE® or ULTRASE® MT to Scandipharm along with a Schandipharm Comprehensive Care Program for CF â„¢ Enrollment Form.
Scandipharm sends the Program participant a one-month supply of: SCANDISHAKE® – 24 shake envelopes and ADEKS® – 60 tablets or a 60ml. Bottle of drops.
Every month, the participant mails in another proof-of-purchase of ULTRASE® or ULTRASE® MT to remain in the Program and receive more SCANDISHAKE® and ADEKS®.
After one full year on the Program, the participant is eligible for a certificate for a FLUTTER® Mucus Clearance Device.

CareFirst for CFSM is an innovative program to help ease the financial burden of infant patients diagnosed with cystic fibrosis.Who is eligible?
Any cystic fibrosis patient or Shwachman-Diamond Syndrome patient in the U.S. or Puerto Rico under two years of age. With a prescription for ULTRASE® Enteric-Coated Microspheres or Enteric-Coated Minitablets

DCI ASSISTANCE
Digestive Care Inc. refers patients not covered by medical insurance to the “Assistance Program” at no cost to the patient. If they are approved, they will then be supplied with a 3-month supply of medications. See details below about the “Assistance Program”:PANCRECARB® Assistance Program: Digestive Care, Inc. offers the Assistance Program to patients who are not covered by medical insurance, Medicaid or other third party payer. The mission of the program is to supply help where it is needed.
Eligibility is determined on a case-by-case basis. To participate in the program, the requesting physician submits a written request outlining the situation and indicates that assistance is needed by the patient. Once the request is approved, the medication will be sent directly to the requesting physician (at no charge) labeled for the specific patient.
Medication must be used as labeled for patient for whom it was requested. Generally, a 3-month supply will be shipped within a week once the request is approved.
